Our Cardiopulmonary Subacute Program is designed for patients who have experienced a significant cardiac or pulmonary event. This program is a bridge to recovery between an acute hospital and going home. In addition, the program also addresses the need for rehab and nursing care after release from the hospital. Our staff supervises and educates patients and their families on a variety of methods to enhance recovery.
Upon admission, residents are evaluated by the center’s specially trained team, including:
• physicians
• nurses
• dietitians
• case managers
• therapists (physical, occupational, speech and respiratory)
• social services professionals
• activities/recreational therapists
• a liaison who assists with your transition from the hospital
The team, the patient and the patient’s family will develop a recovery plan based upon the patient’s medical condition and needs. Patients are instrumental in devloping their goals, which include a safe return to home. To accomplish this, we utilize the “Ready to Go” program, a program designed to take the everyday common tasks the patient must be able to perform safely at discharge and ensure they can perform these tasks themselves prior to returning to home.
Patients will be able to:
• recover in a rehab-focused environment prior to returning home and outpatient rehab
• learn about their disease process and prevention
• improve strength and function
• participate in a progressive walking program
• have decreased re-hospitalizations
Physical, recreational, respiratory, occupational and speech therapies will be incorporated into a recovery plan according to physician orders and the patient’s goals. A progressive walking program will be specifically designed. The goal of the walking program is to increase rehab potential and strength.
The staff will teach the patients how to determine signs and symptoms of complications that may develop after their surgery or hospitalization.

Patients will be monitored for any signs or symptoms of depression, which may occur in patients who have undergone a major health event.
