Welcome
On behalf of our team of caregivers and support staff, thank you for considering our healthcare center. We offer a full range of medical services to meet the individual needs of our residents, including short-term rehabilitative services and long-term skilled care. Working with the resident’s physician, our staff – including medical specialists, nurses, physical, occupational and speech/language therapists, nutritionists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential. Our goal is to provide quality care in a comfortable, compassionate environment, demonstrating on a daily basis our guiding principle of providing Quality Through People.

Overview
Eagle Pond accommodates 128 residents. We are located on a cul-de-sac in a quiet neighborhood. Our beautiful grounds include a fish pond where residents can walk or just relax. Eagle Pond is Medicare and Medicaid certified as well as being accredited by the Joint Commission (formerly JCAHO), which enables us to contract with many insurance companies.
Eagle Pond offers a comprehensive therapy program for our residents through our in-house staff of physical, occupational and speech therapists. Eagle Pond also offers an outpatient therapy program, enabling our residents who return home an opportunity to continue their therapy with the same staff. The facility is proud to have opened the first Alzheimer’s Special Care unit on Cape Cod, designed for the care of Alzheimer’s disease and related dementias by specially trained staff members. Eagle Pond also provides long-term care and respite services.
Eagle Pond Rehabilitation and Living Center is a 2007 recipient of the American Health Care Association Quality Award.
