OUR CENTER

Kindred Transitional Care and Rehabilitation - Eagle Pond accommodates 128 residents and patients. We are located on a cul-de-sac in a quiet neighborhood in South Dennis, the heart of Cape Cod (approximately 12 miles east of Hyannis).

Our short-term rehabilitation and skilled nursing caters to the patient well enough to leave the hospital but not sufficiently recovered to return home. Our goal is to transition patients from hospital to home, safely, and as quickly as possible.

Our specialties include: complex-medical/sub acute skilled nursing and rehabilitation, cardiopulmonary recovery, complex wound care, stroke rehabilitation, respiratory therapy and orthopedic rehabilitation. Our in-house staff of physical, occupational and speech/language therapists are available seven days a week. We also offer an Orthopedic Reservation Program, which allows you to secure your placement for recovery in advance of orthopedic hospital procedures.

Fifteen private rooms are available on our short-term unit. All rooms, private and semiprivate, are spacious with large windows, flat screen televisions with cable, bedside phones and in-suite bathrooms.

In addition to short-term care, we also offer long-term 24-hour skilled nursing and a secure unit for those with Alzheimer’s and dementia. Eagle Pond is Medicare and Medicaid certified and is accredited by the Joint Commission, which enables us to contract with many insurance companies.
